Spay Neuter Initiatives Application Form

Terms of Eligibility and Processes

 

Please read carefully before submitting your application.

This program is not free. It is a lower-cost, subsidized spay/neuter program for the companion animals of residents in Durham Region.  

Please be aware, that as with any surgery, there are some risks with spay and neuter surgeries.  However, the benefits for your companion animal far out-weigh the risks. Complications are very rare but do happen.

Please understand that any unforeseen issues that arise due to complications during surgery or after cannot be covered by this program. You will be responsible for the costs of surcharges (i.e. Pregnant and/or Pyometra spays, Cryptorchid neuters, Hernia repairs).  Charges for any tests or other additional procedures you request from the veterinarian are also your financial responsibility.

All Proof of Income information will be destroyed after completion of all veterinary procedures.  

  • The applicant must be 18 years of age or older and must be a permanent resident of Durham Region.
  • The family annual household income should not exceed the Living Wage as calculated by the Ontario Living Wage Network (listed later in the application form.)
  • Applications must be completed in full. Incomplete applications will not be reviewed. You will not be contacted if your application is incomplete.
  • Spay Neuter Initiatives Association will keep all applications confidential, and will not share information with any other party.
  • If your application is approved, the subsidy rate you are given is non-negotiable. All assessments are done on an individual basis, and any subsidy granted by Spay Neuter Initiatives is to remain confidential.
  • Applications will be kept on file for a period of three months only. If we are unable to make contact within that time, your application will be null and void and you will be required to submit a new application.
  • If your application is declined, you will be notified by email. If your application is declined, you may apply again at a later date if your financial circumstances change.
  • The maximum number of applications is 2 pets per family per year. Separate applications are required to be filled out for each animal.
  • Any issues that arise due to post operative complications will NOT be covered by this program. You are responsible for the cost of surcharges (obese, mature, in heat, or pregnant pet). Charges for any tests or other additional procedures you request from the vet are also your responsibility. 
  • It is at the discretion of the participating veterinarian whether your pet is a candidate for spay/neuter surgery. If your companion animal is deemed unfit for surgery by the veterinarian, please notify Spay Neuter Initiatives.
  • Up-to-date vaccinations may be required either before or during surgery.

 

The application process is as follows:

  • Upon receipt of the application and supporting proof of eligibility registered to the pet guardian, we will contact you either by email or phone within 48 to 72 hours.
  • We will go over your application and supporting information and answer any questions you many have.  We will explain next steps, determine what dates and times will work for you and what you can expect.  We will also discuss your total fee at this time.
  • Upon payment in full via etransfer to spayneuterinitiatives@gmail.com you will be notified of an appointment date with one of our accredited Durham Region veterinarians nearest to your home.
  • Please remember space in our program is limited, so we ask that you try to accommodate the first date that is offered to you.  Otherwise, it may be difficult to reschedule in a timely manner.
  • Cancellation or postponement for any reason requires at least a 48-hour minimum notice and may be subject to a cancellation fee.
  • We will confirm receipt of payment and email to you “Pre and Post Operative Instructions”.  Please read these instructions for your pet.  These instructions are also located on our website for your information.

 

Income Eligibility Information

Total income of one household member not to exceed…………$37,492

Total income of two household members not to exceed…………. $48,204

This number is calculated using the Ontario Living Wage Network's February 2023 statistics for Durham Region.

For each adult income in the household, please provide a copy of the current, official Canada Revenue Agency Notice of Assessment (NOA) showing total income (line #150).

Upon completion of all veterinary procedures the tax assessment(s) will be destroyed.
